Under the direction of the Chief Operations and Compliance Officer, assists patients and families to navigate the healthcare system and develop a palliative plan of care. Ensures nurse navigation services are provided in accordance within industry standards of practice and within policies and procedures of Yolo Hospice and Yolo Care. Ensures documentation of all activities related to palliative care services and navigation activities are accomplished in a timely manner and made available to staff to ensure continuity of patient care. Candidate must possess and have demonstrated experience managing and coordinating timely assignments among various clinical team members and service providers.

SUMMARY OF ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Completes appropriate nursing assessments to include collecting and analyzing all appropriate medical records inclusive of all healthcare history and physical assessments needed to establish a palliative plan of care.
Facilitates the development of the patients’ interdisciplinary plan of care and collaborates with all core disciplines (medicine, nursing, social work, spiritual care, counseling, volunteers and home health care services), including patient and families to identify goals for each patient.
Monitors patient medications and communicates with appropriate medical personnel regarding medication, durable medical equipment and medical supplies.
Understands and utilizes Universal Precautions in the provision of all patient care.
Provides or arranges for all necessary nursing visits.
As patient/family needs or goals are assessed and /or change significantly, the RN shall:
Communicate with the Attending Physician or designee regarding changes in condition and emergent patient needs; facilitates patient care needs within the comprehensive healthcare system and documents all physician recommendations and healthcare navigation communications.
Assess patient needs and collaborate with the interdisciplinary team for changes in level of care and adjustment of acuity rating.
According to established interdisciplinary plan of care and nursing assessment, provides ongoing oversight of physical and emotional care needs of patients’ needs through teaching support skills for patient and family/caregivers and referring to appropriate disciplines.
Provides education regarding medications, activity, diet, symptom management, safety, expected disease progression and potential end of life process in accordance with the patient’s plan of care and Attending Physician collaboration.
Ensures all documentation is complete and submitted in a timely manner according to Yolo Hospice/Yolo Care policy and procedure; documentation must be accurate, reflect nursing assessments and interventions and goals according to individualized plan of care and industry standards of care.
Consistently completes and updates each patient’s plan of care to reflect problems, plans, actions, and goals are accurately and clearly stated and written in accordance with Yolo Hospice and Yolo Care and industry standards of practice.
Collaborates with Attending Physician and all healthcare system providers to insure cohesion in the patient’s plan of care.
RN identifies personal educational needs and seeks and participates in professional educational activities and opportunities to meet these needs.
Graduate of an accredited school of nursing with Bachelor’s degree in nursing and Masters in Nursing preferred.
California RN license required.
Minimum two years of professional nursing experience in hospice and palliative care strongly preferred.
Minimum two years of professional experience serving as a palliative nurse navigator is preferred.
Demonstrated management experience leading and coordinating various assignments among team members and service providers is a must.
Must be knowledgeable, at minimum, about Hospice Medicare Conditions of Participation and California Hospice Licensing Standards.
Program development experience with strength in designing and delivering educational training materials is preferred.
Excellent communication skills, both verbal and written, are required. Public speaking experience is preferred.
Ability to work effectively with community physicians and other health care providers (all disciplines) toward specified goals is required.
Must be self-motivating individual who can work autonomously, set own priorities, organize work, take initiative and make independent decisions relating to job tasks.
Must be computer literate in Microsoft Word; Excel and Power Point software preferred.
Ability to successfully meet agency background screening requirements.
PHYSICIAL AND OTHER JOB SPECIFICATIONS
Ability to communicate well in English (spoken and written) is required. Ability to communicate in Spanish, Russian, and/or Chinese is a plus.
Able to use computer keyboard in order to perform job duties.
Able to lift and carry up to 15 pounds.
Must be willing and able to flex hours to meet occasional appropriate business needs during evenings or weekends.
If driving on Yolo Hospice business, must have a valid California driver’s license and state-required automobile insurance.
